Learning Objectives

At the conclusion of this activity, participants should be able to:

  • Describe the needs of the long-term care and hospital industries, and how clinicians can market their services to improve transitions of care in these settings
  • Identify opportunities in a patient's journey through the system in which to provide improved patient outcomes and lower health care costs by optimizing pharmaceutical care
  • Distinguish between value-based reimbursement and other reimbursement models, and discuss how evolving reimbursement models will change how pharmacy services are provided in the future
  • Construct a roadmap with strategies to improve transitions of care throughout the different settings in which a long-term care patient might move
  • Identify and evaluate key services that senior care and other pharmacists can provide in post-acute care settings to improve patient outcomes
  • Apply metrics to evaluate outcomes of pharmaceutical care in older adults
  • Develop strategies to improve communication and participation as a pharmacist on the interdisciplinary team

Continuing Pharmacy Education


The American Society of Consultant Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This activity, ACPE UAN: 0203-0000-15-008-L04-P is approved for 6.0 contact hours.

To receive continuing pharmacy education (CPE) credits, participants must attend the activity in its entirety and complete the evaluation online through ASCP's Professional Development Center. Evaluations must be completed within 30 days of the activity. ASCP reports CPE credits to ACPE and NABP via the CPE Monitor system. ASCP has submitted (pending approval from the FL Board of Pharmacy) this activity for Florida consultant pharmacist recertification credits.

Continuing Education for Nursing Home Administrators and Assisted Living Managers


This program is approved through the Beacon Institute for 6 hours of continuing education credits for nursing home administrators and assisted living managers by the National Continuing Education Review Service (NCERS) of the National Association of Boards of Examiners of Long-Term Care Administrators (NAB).

  • 6 credit hours of continuing pharmacy education (CPE)
  • 6 credit hours of continuing education for nursing home administrators and assisted living managers
